REGISTER WITH THE SURGERY?

When registering with the practice, we would request that you bring your medical card with you to assist our administrative work. You may also be asked for additional proof of identification. We would also request that new patients complete a questionnaire that will provide us with details of your health and present medication. You will be registered with the practice. However, you will be asked to nominate a preferred practitioner, which will assist with continuity of care. Patients will also have the opportunity to see any practitioner available.

You will be offered an appointment to discuss your health with your new practitioner. There may be several forms to complete during this registration process - please ensure that the details are accurate.

CHANGE MY PERSONAL DETAILS?

Please let us know promptly if any of your personal details change. The practice has occasionally encountered problems in contacting patients regarding health matters because the address or telephone number we have on record is out of date.
